Educational Permit Renewal Instructions

All Doctors of Osteopathic Medicine (D.O.) participating in graduate medical training in West Virginia must have a full license or an Educational Permit issued by the West Virginia Board of Osteopathic Medicine. A physician who holds a full license or previously held a full license in any jurisdiction does not qualify for an Educational Permit and must apply for a full license with the Board.

An Educational Permit is not a full license to practice osteopathic medicine and surgery in West Virginia. An Educational Permit only authorizes the holder to practice osteopathic medicine and surgery within his or her training program. To practice osteopathic medicine and surgery in West Virginia outside of a training program, a physician must be licensed by the Board.

Educational Permits are issued for up to a one-year period. Educational Permits expire on June 30th of every year. Educational Permits are renewable for as long as the holder continues to participate in graduate medical training in West Virginia. Educational Permits automatically expire and become void if the holder leaves the training program for any reason.

You may access the renewal application for an Educational Permit by using the Continue button at the bottom of the page. In addition to the completing the application, you must:

1. Submit a letter of good standing from your training program; and
2. Submit a $100 application fee plus a 2.25% processing fee.

You must submit the renewal application, letter of good standing, and fees to the Board by June 30th. You cannot continue in your training program until all required documents and fees have been received and processed.
