Controlled Substance License Application

To apply for a controlled substance license, please access the online application form by using the Submit & Continue button at the bottom of the page. In addition to completing the application, you will also need to submit an application fee of $15.00 plus a processing fee of 2.25% if paying via credit card.

While you are in the application process, please do not use the back button in your browser. If at any time you wish to exit the application process, please click the "Cancel & Exit" button located at the bottom of the page.

If you have not logged in before your initial password will be your last name and last 4 digits of your SSN combined with no spaces. Example: Test1234
