D.O. Application

The online application may be accessed below by clicking on the "Begin Application" button at the bottom of the page. Failure to follow the below steps may result in delays in processing your application.

In addition to completing your application, you will need to complete the following steps, AND the board will need to receive the items listed below before your application can be processed.

As part of completing your online application, you will need to upload the following documents:

  1. Medical School Diploma:
    • Upload a copy of your Medical School Diploma.
  2. Malpractice Liability Form:
  3. Color Photo:
    • Upload a recent (within 60 days) passport quality color Photograph.
  4. Application Fee:
      Submit a $400.00 Application Fee and 2.25% processing fee.

After submitting your online application, you will need to have the following items sent to the board office:

  2. Medical School Verification:
    • The Board must receive a verification form from every medical school you have attended.
      1. Login to your D.O. Dashboard using your last name and the password you created while filling out your application.
      2. Under Online Forms, click on the Medical School Verification Invite box.
      3. Fill in the Name of the Medical School, the Name of a contact person from your school, and the Email of the contact person from your school.
      4. Click Submit to send the form to your Medical School.
      5. Your school will fill out and return the form to the board.
  3. Post Graduate Education Verification:
    • The board must receive verification of ALL postgraduate education you have received.
      1. Login to your D.O. Dashboard using your last name and the password you created while filling out your application.
      2. Under Online Forms, click on the Postgraduate Education Verification Box.
      3. Fill in the Hospital Name, the Name of a contact person from your program, and the Email of the contact person from your program.
      4. Click Submit to send the form to your Post Graduate Education Program.
      5. Your program will fill out and return the form to the board.
  4. License Verifications:
    • The board must receive a license verification from EVERY STATE in which you currently hold or have held a license. This includes training licenses:
      • Visit www.veridoc.org to request verification of your licensure online.
      • OR
      • If the state in question does not participate in verification through Veridoc, contact that state’s licensing board directly and request that verification be sent to the West Virginia Board of Osteopathic Medicine.
  5. Proof of Name Change (if applicable):
    • Mail or upload name change documentation (e.g. marriage certificate or divorce document).
    • Note:  If your name has changed, and any of your licensure documentation (internship certificate, medical school diploma, other licensure certificates, etc.) shows a different name, you must provide documentation of this change (e.g. marriage certificate or divorce document).
  6. Background Check:
    • All new applicants must obtain a Criminal Background Check.
    • To obtain a background check:
      1. Present to any IdentoGO Center
        • A listing of available centers can be found at: www.identogo.com 
        • You will need Service Code: 228QBF in order to process your fingerprints.

      Advise the background center that reports are to be sent to the WV Board of Osteopathic Medicine.

      Please Note:
      1. There will be a fee of $47.25 charged by the background check center.
        • Additional fees may apply for out-of-state applicants.
      2. Both State and Federal reports are required.

 

Once your application file is complete, your file will be processed and reviewed.

  • Based on that review, you will be informed whether an interview is required before the board may issue your license.
  • Your application status can be checked by logging back into your account.
  • Items sent to the board office must be processed before they will be reflected on your account.
Please do not call the board office to confirm receipt of documents sent to the board unless:
  1. You have allowed 5 business days for processing of your documents after receipt
  2. AND
  3. The documents are not reflected on your application status page.
PLEASE NOTE:
  • Board staff cannot process your application for licensure until they receive all required documentation from you and outside sources.
  • The board has no control over the timeframe in which other entities process requests for information.
  • It may take some time for the board to receive Verification of your education and/or verification of your licensure in other states.
