Who Can File a Complaint?
Any person, medical peer review committee, firm, corporation, member of the Board, or public officer may make a complaint to the Board which charges an osteopathic physician or osteopathic physician assistant with a violation of WV Code '30-14-1 et seq. or of the rules of the Board.

Records Release Authorization
If you are the patient at issue in the complaint, please download and fill out the records release form. The form will need to be notarized. There is a place within the form for the completed form to be uploaded with the complaint information. Otherwise you may either fax or email the completed form back to the board.
