PLLC Application Process

You may access the online application for Professional Limited Liability Companies by using the Continue button at the bottom of the page. With the application, you will also need to:

• mail or upload a copy of the Articles of Organization for the Professional Limited Liability Company (PLLC)
• mail or upload documentation of at least one million dollars of liability insurance on the PLLC.
• submit a $200.00 application fee
• submit a processing fee that is 2.25% of the total fee
