Controlled Substance License Renewal

Registration to dispense controlled substances can be renewed using the online renewal form by using the Submit & Continue button at the bottom of the page. In addition to completing the application, you will also need to submit a renewal fee of $15.00 plus a processing fee of 2.25% if paying via credit card.

The application and fees must be submitted to the West Virginia Board of Osteopathic Medicine by June 30th.

While you are in the renewal process please do not use the back button in your browser. If at any time you wish to exit the renewal process, please click the "Cancel & Exit" button located at the bottom of the page.

If you have not logged in before your initial password will be your last name and last 4 digits of your SSN combined with no spaces. Example: Test1234
